Brief Summary: On December 5, 2023, the M&C approved the Community Partnership Program (CPP) process that would provide General Fund grants to eligible community partner agencies through a competitive process, in order to support achieving the goals and priorities identified in the FY23-25 Strategic Plan that specifically benefit ACC residents. Managed by the Housing and Community Development Department (HCD), the first iteration of the CPP RFP process took place in Q3 FY24. M&C awarded funds on May 7, 2024 to the agencies outlined below.

12 participants will exit into stable housing, actively engage in case management services, and maintain or elevate their pathway status.

Sparrow's Nest - Breaking the Cycle Program

20 participants will obtain employment, housing, and financial literacy skills through course completion.

Acceptance Recovery 

Center – ARC Indigent Funds Program

2 high-risk and high-need individuals will receive reduced admission fee to 12-month residential substance abuse disorder recovery program; 3 families will receive childcare support to attend evidence-based peer-support services.

Athens Area Homeless Shelter – Shelter Programs

Will host 50 family enrichment sessions and 12 community resource events; 35 families will increase potential for housing; 20 families receive low-barrier shelter and services; 7 families will increase earning power.

Athens Model Neighborhood Health Center – 

Behavioral Health Program

Licensed Clinical Social Worker (LCSW) will: provide visits to 192 indigent patients, complete 1,752 LCSW experienced hours, provide 768 visits for Clarke County School District (CCSD) students.

Georgia Conflict Center - Restorative Justice 

Diversion Program

40 youth will participate in family outreach, Restorative Justice Conference, family outreach, and maintain connection to family and positive supports after program in community; 36 participants in RJD program and 9 participants in CCSD referral program will be highly satisfied with RJD process and outcome.

Divas Who Win – School Dayz Program

30 youth will increase knowledge of anti-trafficking and substance abuse, personal development and leadership skills. 30 youth will see improvements in: mental health and wellbeing, academic achievements and future opportunities, and feelings of belonging to a safe and supportive community.

Juvenile Offender 

Advocate – JOA Program

10 youth will increase academic success; 8 youth will reduce recidivism rate; 50 youth will enhance financial literacy; 15 youth’s family stability will improve; 50 youth will experienced positive community engagement.

Project Safe – High Risk Team Advocates

230 participants will be informed about resources; 173 high-risk survivors' documented notes will be provided; 53 engagements will be logged and tracked with ACCPD and Caseworthy; 117 high risk survivors will learn new ways to manage safety; 186 high risk survivors will utilize resources.

United Way of Northeast Georgia – 2-1-1 Program

810 individuals will be referred; 10 new agencies will be added to 211 referral and service database; 172 agencies will update listings

Athens Area Diaper Bank - Diaper Distribution 

via Healthcare Orgs

2,700 children will receive diapers. 90% of them will have improved health and general well-being improved; 90% of families will be connected to other resources through program.

Envision Athens – 

Operations

Healthy Athens: will hold 12 meetings with 80 agencies; will train 300 people on NARCAN and supplied with NARCAN; 200 people will attend Substance Use Disorder Summits. Abundance Athens: will hold 12 meetings with 30 agencies; 20 agencies will connect to Food Bank of NEGA; 20 youth will be served through Lunch, Learn and Love program. Create Athens: will host 12 meetings with 20 agencies; will finalize, publish and market Create Athens directory and a Create Athens Teaching registry. Educate Athens: will hold 4 meetings with 20 agencies; 20 employers will receive training on 2nd Chance Employment. House Athens: will hold at last 4 meetings with 20 agencies; will support development of Athens Homeless Coalition.

Athens Land Trust – Young Urban Builders Program

2,100 hands-on work experience hours will be provided for 6 YUB; 6 YUB will complete OSHA, CPR/First Aid, and Building Performance Institute Building Science Principles Certificates; 7 LMI Home-owners will receive a total of 30 repair/weatherization/rehab services.
